2. I am a new patient, what materials do I need to bring to my first visit?
In order to make your first appointment smooth and quick please have your insurance information, form of I.D. (driver’s license, passport, state identification card), any previous medical records necessary.

3. What are your cancellation/late arrival policies?
We understands situations change and last minute emergencies happens, but please notify us ASAP via phone for any cancellations or changes. Our intent is to give each patient the time and attention they deserve. Your time is valuable and we will try our best to stay on schedule, if you are late to your appointment, we will make it our best effort to see you the same day, but on occasion, may be asked to wait or re-schedule.
4. Is parking available?
Our offices offer valet parking for $3/per hour up to 3 hours. Any stay longer than 3 hours may incur a fee by our third-party valet service. There is also limited street parking available curbside.
5. How can I schedule a televisit?
First visit needs to be established in person. Once an in-person visit has been completed, you may request a televisit by reaching us at 718-261-9300. Media content privacy statements must be signed and agreed beforehand.
